Create a work activity first. You can develop the Risk Assessment from it later.

1 Activity Title

Provide a short, clear title for the activity. Examples: Tree Pruning, Warehouse Forklift Operation, Office DSE Work.

2 Activity Description

Provide a detailed description to help generate an accurate OHS Risk Assessment. Include:

  • What the activity involves and how it is performed (tasks/processes).
  • Who conducts the activity (roles, headcount, limitations like young workers, pregnancy, disability).
  • Tools, equipment, or machinery used (ladders, power tools, vehicles).
  • Environment (indoors/outdoors, public areas, confined spaces, heights).
  • Interactions with others (public, contractors, other workers).
  • Specific conditions or hazards (weather, noise, chemicals, heavy lifting, traffic).
Example: Tree pruning in a public park by two trained arborists using chainsaws and a cherry picker. The activity occurs outdoors, 10 meters above ground, near a public walkway where pedestrians may pass. One worker is a young apprentice. Weather conditions may include rain, and chainsaw noise is a concern.
